Random variation drives a critical bias in the comparison of healthcare-associated infections

Objective:
To evaluate random effects of volume (patient days or device days) on healthcare-associated infections (HAIs) and the standardized infection ratio (SIR) used to compare hospitals.
Design:A longitudinal comparison between publicly reported quarterly data (2014–2020) and volume-based random sampling using 4 HAI types: central-line–associated bloodstream infections, catheter-associated urinary tract infections, Clostridioides difficile infections, methicillin-resistant Staphylococcus aureus infections.
Methods:Using 4,268 hospitals with reported SIRs, we examined relationships of SIRs to volume and compared distributions of SIRs and numbers of reported HAIs to the outcomes of simulated random sampling. We included random expectations into SIR calculations to produce a standardized infection score (SIS).
Results:Among hospitals with volumes less than the median, 20%–33% had SIRs of 0, compared to 0.3%–5% for hospitals with volumes higher than the median. Distributions of SIRs were 86%–92% similar to those based on random sampling. Random expectations explained 54%–84% of variation in numbers of HAIs. The use of SIRs led hundreds of hospitals with more infections than either expected at random or predicted by risk-adjusted models to rank better than other hospitals. The SIS mitigated this effect and allowed hospitals of disparate volumes to achieve better scores while decreasing the number of hospitals tied for the best score.
Conclusions:SIRs and numbers of HAIs are strongly influenced by random effects of volume. Mitigating these effects drastically alters rankings for HAI types and may further alter penalty assignments in programs that aim to reduce HAIs and improve quality of care.

Background:
During the Randomized Assessment of Rapid Endovascular Treatment (EVT) of Ischemic Stroke (ESCAPE) trial, patient-level micro-costing data were collected. We report a cost-effectiveness analysis of EVT, using ESCAPE trial data and Markov simulation, from a universal, single-payer system using a societal perspective over a patient’s lifetime.
Methods:Primary data collection alongside the ESCAPE trial provided a 3-month trial-specific, non-model, based cost per quality-adjusted life year (QALY). A Markov model utilizing ongoing lifetime costs and life expectancy from the literature was built to simulate the cost per QALY adopting a lifetime horizon. Health states were defined using the modified Rankin Scale (mRS) scores. Uncertainty was explored using scenario analysis and probabilistic sensitivity analysis.
Results:The 3-month trial-based analysis resulted in a cost per QALY of $201,243 of EVT compared to the best standard of care. In the model-based analysis, using a societal perspective and a lifetime horizon, EVT dominated the standard of care; EVT was both more effective and less costly than the standard of care (−$91). When the time horizon was shortened to 1 year, EVT remains cost savings compared to standard of care (∼$15,376 per QALY gained with EVT). However, if the estimate of clinical effectiveness is 4% less than that demonstrated in ESCAPE, EVT is no longer cost savings compared to standard of care.
Conclusions:Results support the adoption of EVT as a treatment option for acute ischemic stroke, as the increase in costs associated with caring for EVT patients was recouped within the first year of stroke, and continued to provide cost savings over a patient’s lifetime.

Summary
Reward Deficiency Syndrome (RDS) is an umbrella term for all drug and nondrug addictive behaviors, due to a dopamine deficiency, “hypodopaminergia.” There is an opioid-overdose epidemic in the USA, which may result in or worsen RDS. A paradigm shift is needed to combat a system that is not working. This shift involves the recognition of dopamine homeostasis as the ultimate treatment of RDS via precision, genetically guided KB220 variants, called Precision Behavioral Management (PBM). Recognition of RDS as an endophenotype and an umbrella term in the future DSM 6, following the Research Domain Criteria (RDoC), would assist in shifting this paradigm.

A national need is to prepare for and respond to accidental or intentional disasters categorized as chemical, biological, radiological, nuclear, or explosive (CBRNE). These incidents require specific subject-matter expertise, yet have commonalities. We identify 7 core elements comprising CBRNE science that require integration for effective preparedness planning and public health and medical response and recovery. These core elements are (1) basic and clinical sciences, (2) modeling and systems management, (3) planning, (4) response and incident management, (5) recovery and resilience, (6) lessons learned, and (7) continuous improvement. A key feature is the ability of relevant subject matter experts to integrate information into response operations. We propose the CBRNE medical operations science support expert as a professional who (1) understands that CBRNE incidents require an integrated systems approach, (2) understands the key functions and contributions of CBRNE science practitioners, (3) helps direct strategic and tactical CBRNE planning and responses through first-hand experience, and (4) provides advice to senior decision-makers managing response activities. Recognition of both CBRNE science as a distinct competency and the establishment of the CBRNE medical operations science support expert informs the public of the enormous progress made, broadcasts opportunities for new talent, and enhances the sophistication and analytic expertise of senior managers planning for and responding to CBRNE incidents.
Patterns of Dental Evolution in Early Eocene Anaptomorphine Primates (Omomyidae) from the Bighorn Basin, Wyoming

The subfamily Anaptomorphinae contains the oldest and most generalized members of the tarsier-like primates and is the basal group of the extinct family Omomyidae. The best and most continuous record of anaptomorphine history is from rocks of early Eocene (Wasatchian) age in the Bighorn Basin of northwest Wyoming where eight genera and 14 species are recognized. Three of these species are new (Teilhardina crassidens, Tetonius matthewi, Absarokius metoecus), and four other new species are described from elsewhere (Tetonius mckennai, Absarokius gazini, A. australis, Strigorhysis huerfanensis). Teilhardina tenuicula and Absarokius nocerai are new combined forms. Absarokius noctivagus is considered to be a synonym of A. abbotti, and Mckennamorphus is a synonym of Pseudotetonius.
The evolution of dental characters in three principal morphologic clades of anaptomorphines from the Bighorn Basin is documented with the aid of numerous new specimens (75% of the sample is new) and with precise stratigraphic data. These major clades are Teilhardina–Anemorhysis, Tetonius–Pseudotetonius, and Absarokius. In each of these clades, evolution appears to have occurred gradually. In the first two clades it was mainly anagenetic, although each one included a minor branching event. In Absarokius, evolution was instead characterized by cladogenesis followed by continued (and continual) anagenetic change in each of the new lines. Anagenetic gradual evolution produced the new genus Pseudotetonius (from Tetonius) and possibly Anemorhysis (from Teilhardina). Similarly, the Absarokius metoecus lineage probably gave rise to late Wasatchian–early Bridgerian Strigorhysis. Evolution from Tetonius to Pseudotetonius has been clarified by establishment of five arbitrary stages of evolution (Tetonius–Pseudotetonius intermediates). Estimates of relative proportions of time represented by paleosols in different parts of the Willwood section suggest that cladogenetic speciation in Absarokius was almost certainly more rapid than anagenesis in Tetonius–Pseudotetonius.
Anagenetic character evolution and speciation in the anaptomorphine primates was typified first by increase, then decrease in variability, which resulted in measurable apomorphic morphologic change. Cumulation of changes of this sort created more extensive differences of importance at the species and genus levels. Introduction of changing character states and their tempo was staggered temporally, and new characters (and new taxa) are least separable from their antecedent states when they first appear. This evidence is in sharp contrast to predictions of the punctuated equilibria model of evolution. Because the emergence of diagnostic characters occurs gradually (in evolutionary terms) and not all at once (in temporal terms), and because diagnostic characters are the essence of the diagnosis (and thereby taxonomy), the implications of gradual evolution for both systematic paleontology and biostratigraphy are profound. Stasis exists in the evolution of individual characters over certain periods, but this study offers no evidence supporting either organismic stasis or even stasis in the dental evolution of the Anaptomorphinae over a period of about 4.8 million years.
Analysis of PEDOT:PSS Films After Sulfuric Acid Treatment on Silicon and Fused Silica using FT-IR and UV-VIS

Thin films of organic semiconductor PEDOT:PSS deposited onto silicon and fused silica substrates. These films were then treated with sulfuric acid (H2SO4) for various amounts of time (i.e., 10, 20, 40, 60, and 80 minutes). Preliminary results obtained with FT-IR, UV-VIS, and Van DerPauw conductivity methods suggest that the H2SO4 removes the PSS isonomer from the PEDOT:PSS system. This PSS removal also induces a decrease in film thickness.
Early Eocene Paromomyidae (Mammalia, Primates) from the southern Bighorn Basin, Wyoming: Systematics and evolution

We present a critical review of the alpha taxonomy and evolution of Eocene North American paromomyid primates, based on analysis of more than 570 stratigraphically controlled dental and gnathic specimens from the early Eocene of the southern Bighorn Basin, Wyoming (Wasatchian, Willwood Formation). In addition to documenting numerous previously unpublished specimens of known taxa (including deciduous teeth), we also describe a new species, Phenacolemur willwoodensis n. sp., from the upper part of the Willwood Formation (Wa 5 and 6). The new species is intermediate in size between Phenacolemur simonsi and Phenacolemur citatus and has both primitive features (e.g., retention of m2–3 paraconids, relatively long molar trigonids) and derived traits (e.g., relatively reduced paraconid on m1 and no p4 paracristid, unlike Paromomys). Overall patterns of dental evolution in southern Bighorn Basin paromomyids provide some support for previously hypothesized periods of faunal change (Biohorizons). In particular, Phenacolemur praecox evolves into the similarly sized but morphologically distinct Phenacolemur fortior at Biohorizon A, and P. fortior is replaced by P. citatus just below Biohorizon B. Two taxa previously believed to have become extinct at Biohorizon A (Ignacius graybullianus, P. simonsi) are shown to have persisted about a million years longer than previously thought. The Bighorn Basin paromomyids are of general interest in comprising a very dense sample that allows for the study of patterns of evolution against the backdrop of well-understood patterns of change in other mammalian lineages, and in climatic variables.

The free market-based policies of the corporate community model have skewed economic development across the South. For many small, rural communities, the consequences of global capitalism have resulted in declining real wages, high underemployment, and increasing rates of income inequality. Backed by recent scholarship and grassroots movements that suggest that both civic engagement and the presence of smaller-scale, locally controlled enterprises can help determine whether communities prosper or decline, this paper explores the links between social structure and rural development in the South. The goal is to expand our understanding of civic community theory as an alternative to the neoclassical economic model of development. Using a local problem-solving framework, we suggest that a departure from the traditional, neoclassical path of development is in order. We conclude that rural policy makers must establish a role for civic community in the rural development process if they wish to protect the welfare of workers and communities, while increasing the prospects of economic growth with prosperity.

Avian diet selection is hypothesized to be sensitive to seasonal changes in breeding status, but few tests exist for frugivorous tropical birds. Frugivorous birds provide an interesting test case because fruits are relatively deficient in minerals critical for reproduction. Here, we quantify annual patterns of fruit availability and diet for two frugivorous hornbill (Bucerotidae) species over a 5.5-y period to test for patterns of diet selection. Data from the lowland tropical rain forest of the Dja Reserve, Cameroon, are used to generate two nutritional indices. One index estimates the nutrient concentration of the diet chosen by Ceratogymna atrata and Bycanistes albotibialis on a monthly basis using 3165 feeding observations combined with fruit pulp sample data. The second index is an estimate of nutrient concentration of a non-selective or neutral diet across the study area based on tree fruiting phenology, vegetation survey and fruit-pulp sample data. Fifty-nine fruit pulp samples representing 40 species were analysed for 16 nutrient categories to contribute to both indices. Pulp samples accounted for approximately 75% of the observed diets. The results support expected patterns of nutrient selection. The two hornbill species selected a diet rich in calcium during the early breeding season (significantly so for B. albotibialis in July and August). Through the brooding and fledging periods, they switched from a calcium-rich diet to one rich in iron and caloric content as well as supplemental protein in the form of invertebrates. Calcium, the calcium to phosphorus ratio and fat concentration were the strongest predictors of breeding success (significant for calcium and Ca:P for B. albotibialis in June). We conclude that hornbills actively select fruit based on nutritional concentration and mineral concentration and that the indices developed here are useful for assessing frugivore diet over time.

A series of synthetic calcium silicates has been produced comprising nano-sized plates. The starting calcium silicate referred to as nano-structured calcium silicate, NCS, appears itself X-ray amorphous but contains impurities of calcite. These impurities decompose during the dry thermal conversion of the material into wollastonite. NCS can be enriched with calcium or silicon, respectively. The silicon enriched NCS can be hydrothermally transformed into a sheet material related to Ca7Si16O38(OH)2. The X-ray pattern of this material was sufficient to allow the calculation of its unit cell with a comparatively high figure of merit. The calcium enriched NCS can form two very distinctly different ceramic materials upon hydrothermal treatment, one a band material (formed below 200 °C) and the other a rose petal shaped material (formed above 210 °C). The X-ray diffraction patterns could not be resolved. The fit between the observed and calculated patterns was less than 50% as expressed in comparatively low figures of merit (unusually <20), which is attributed to calcium carbonate impurities in the samples disrupting the long range order. The patterns of these calcium enriched samples could be best compared to those of tobermorite or truscotite.
